Judgement, not technique: what Core asks of you

Marta has been supporting Mr Buckley for a month. He walks with a frame, slowly, and manages. This morning his son meets her at the door, flustered: "Dad's slipped down in the armchair, he can't get comfortable — grab his other arm, we'll pull him up in a second."

There it is. The moment this whole course exists for. A kind request, an easy-sounding tug, and two seconds that could hurt Mr Buckley's shoulders and skin, put his son on the floor beside him, and end Marta's working life with one wrong strain of her back.

Let's be plain about what this course is and is not, before anything else. **This course does not teach you how to move people.** No text on a screen can. The way you position your feet, where a sling sits, how a slide sheet is used, how two people co-ordinate a move — these are *practical* skills, learned in person with a trainer, practised until they are yours, and only ever used once your employer has trained you for them and agreed, in writing, that they are part of your role. That rule protects the person, and it also protects you.

What this course does teach is everything *around* the moving: the judgement. When to help and when to stop. What to look at before anyone moves anywhere. What the plan means and what to do on the day the plan no longer fits the person in front of you. What the equipment is, what state it should be in, and who may touch it. How skin gets harmed by bad moving, and how falls are prevented, survived and learned from. And — perhaps most valuable of all — how to say no, warmly and immovably, when someone asks you to move a person in a way you should not.

You did the Foundation course, Mobility and Moving Safely, so you already have the foundations: why movement matters, the basics of a safe environment, encouraging people to do what they can. Core assumes all of that and adds the harder thing — decisions under pressure, in real rooms, with real people asking.

**Why this matters so much — for them.** For the people you support, being moved is not a neutral event. Done well, with their own effort at the centre, movement keeps muscles working, joints free, lungs clear, skin healthy, and confidence alive. Done badly, a single move can bruise skin that has grown paper-thin, wrench a shoulder that no longer glides as it once did, drag skin against fabric hard enough to start damage you will not see for days, and — worse than any of that in the long run — frighten a person. A frightened mover moves less. Moving less makes every part of health worse. One rough, rushed transfer can begin a spiral that never quite unwinds. Gentleness here is not a courtesy; it is a treatment.

**And for you.** Care work asks a great deal of backs, shoulders and knees, and lifting and moving done badly is one of the most common ways people in caring jobs get hurt. Back injuries in particular have ended countless caring careers — not in one dramatic accident, usually, but in an accumulation of "just this once" moments exactly like Marta's doorway. Here is the fact to hold onto: **an adult human being is heavier than anything you would ever agree to lift in any other job.** Nobody would hand you a washing machine and say "just grab the other side". A person who has slipped down a chair is not lighter than a washing machine, and unlike the washing machine they can move unexpectedly, grab you, or panic. Your body is your livelihood. Every rule in this course protects it alongside the person you support.

**The spectrum of help.** Not all "helping someone move" is the same act, and knowing where you are on the spectrum at any moment is the central Core skill:

- **Encouraging and prompting.** The person moves themselves; you provide the reason, the time, the patience, and a clear path. "Take your time; the frame's right in front of you." This is the best kind of help there is, and it is always in your role.

- **Steadying and supervising.** The person moves themselves; you are close, attentive, hands available for light reassurance and balance the way their plan describes — not taking their weight. Walking beside someone with your arm offered is worlds apart from holding someone up.

- **Assisting with equipment and trained techniques.** The person's plan calls for particular equipment or particular assistance — a slide sheet, a handling belt used a specific way, a two-carer standing transfer. These are only ever done *by people trained for that task, for that person*, following the plan.

- **Hoisting and full transfers.** The person's weight is taken entirely by equipment, operated by trained people, usually two of them, exactly per the plan.

The spectrum only moves to the right — toward more help — through proper decisions: an assessment, a plan, training, agreement. It never moves to the right because a morning went wrong and improvising felt quicker.

**The bright lines.** Whatever your training covers, some things are outside everyone's role, always:

- **You never take a person's full weight with your body.** Lifting someone in your arms, "bear-hugging" them out of a chair, having them hang from your neck — every one of these is dangerous for both of you, every time.

- **You never drag or pull a person** — by the arms, under the armpits, by clothing — up a bed, up from a chair, up from the floor. The under-the-arms drag has a long history in old-fashioned care and a long record of hurting shoulders and skin. It has no place anywhere.

- **You never move a person against their will.** Moving and handling is done *with* consent, like everything else. A person who says stop, stops the move — and what happens next is conversation and problem-solving, not more force.

- **You never use equipment you have not been trained on**, and you never improvise equipment. A bedsheet is not a slide sheet. A dressing-gown cord is not a handling belt.

- **You never do a two-person task alone.** If the plan says two, it means two, at three in the afternoon and three in the morning alike.

**Four myths worth burying.** Moving and handling drags some old beliefs behind it, and Core is a good place to bury them.

*"Lifting people is just part of care."* It was said for decades, and a generation of carers paid for it with their backs. Modern care is built the other way round: people are supported to move themselves, equipment takes the weight that must be taken, and nobody's spine is part of the plan. A workplace that treats body-lifting as normal is not old-fashioned; it is unsafe, and it is worth questioning out loud.

*"A strong carer makes the move safe."* Strength is the most overrated quantity in this whole subject. Strong people can still be injured — an awkward angle and a sudden grab do not check your gym record — and strength tempts people into moves that should never happen at all. Judgement, planning and the right kit keep moves safe. Strength mostly keeps gym equipment company.

*"Hoists and equipment are cold and undignified."* Ask someone who has been half-dragged out of a bath by two straining, embarrassed helpers, and then moved by a smooth, unhurried hoist transfer with a carer talking them through it. The indignity is in fear, pain and near-misses — not in machinery. Equipment used well, by trained hands, with warm words, is dignity engineered.

*"Quick is kind."* The rushed transfer that "gets it over with" is the one that frightens, bruises and falls. Slow is kind. Slow is also, over any honest month, faster — because slow does not spend Tuesday afternoon in A&E.

**The law has your back — literally.** You do not need statutes by heart, but know this much: health and safety law in the UK puts firm duties on employers around moving and handling — to assess the risks, to provide the equipment and training the assessment calls for, and not to leave staff improvising with their bodies. It also expects *you* to use the training and equipment provided, to work within what you have been trained for, and to report what is unsafe. And where care is provided by a registered service, the CQC expects safe moving and handling as part of safe care generally. All of which means: when you decline an unsafe lift, insist on the second carer, or report a broken hoist, you are not being difficult. You are doing precisely what the entire framework — legal, professional and regulatory — is built to make you do. The system is on the side of your no.

**Back to Marta's doorway.** Watch her apply all of this in thirty seconds, because it is genuinely simple once the thinking is in place. Is Mr Buckley in danger right now? No — he is uncomfortable, not trapped, breathing fine, not in pain beyond annoyance. So there is no emergency to justify anything rushed. Is "pull him up by the arms" inside anyone's role? No — it is a drag lift on a man with thin skin and stiff shoulders. Can Mr Buckley move himself with encouragement? Quite possibly — with time, the right prompts, perhaps his frame and a firmer cushion, many people can reposition themselves. That is where Marta starts. And if he genuinely cannot, this becomes a "his needs have changed" conversation — the plan, the family and possibly an assessment need to catch up, today, through a phone call rather than through two strained backs. Marta's answer to the son is the course in one breath: "Let's not pull him — that can hurt him and us. Let me see what he can do himself first, and if he's stuck, I'll get the right help sorted."

**Consent and dignity ride along on every move.** It is easy, amid weights and risks, to forget that being moved is intensely personal. At Core level your standard is that the person is the *author* of the move, not its object. Tell them what is proposed, every time, in words they follow — even when confusion means they may not hold it long; especially then. Agree the signal to start. Move at their pace, not the visit schedule's. Keep them covered and comfortable. Talk *to* them during the move, never over them to your colleague about the weekend. Afterwards, ask how it was — the answer trains you for next time. People allow their bodies to be helped in proportion to how safe the last helping felt.

**What "getting it right" quietly builds.** Do this work well for a few months and something compounding happens. The person trusts you, so they relax; relaxed bodies move more easily and hurt less. They keep doing what they can, so what they can do shrinks more slowly — sometimes it even grows. They fall less, because rushed, frightened moving is where many falls live. Their skin stays whole. Your back stays whole. Not one of those outcomes came from strength. All of them came from judgement — which is exactly why judgement, not muscle and not memorised technique, is what this course trains.

**Near-misses count here too.** One habit to carry from your other training straight into this subject: the move that *almost* went wrong is information, not luck to be pocketed. The foot that slipped but caught, the brake that was not on until the last check, the sling clip that looked odd and turned out fine, the moment two of you realised mid-transfer that you had different ideas of the destination — every one of these is tomorrow's accident introducing itself politely. Report near-misses the way you report the real thing: plainly, promptly, without embarrassment. Workplaces learn from the near-misses they hear about and repeat the ones they do not. The same goes for the household you work in: "that rug nearly had us today" said out loud to the family is prevention; said only in your head, it is a countdown.

**Three doorway moments to test yourself on.**

*One.* A new client's daughter shows you around: "Mum can't really walk any more, so we just lift her between us — I'll show you how we do it." — However long the family has managed this way, you joining a two-person body lift is not on any spectrum you belong to. Warm no; genuine sympathy for how hard they have worked; and escalation with energy, because a household lifting a person by hand needs an assessment and probably equipment and training — help that actually helps. Until then, you support what Mum can do herself, and you record and report what you found.

*Two.* Mr Osei, recovering well, waves away the frame the plan insists on: "I'm past that thing." — Encouraging news, maybe — and not yours or his to settle between you at the kitchen door, because the plan exists for reasons you may not fully see. Today, warmly hold the plan ("humour me while it's official") while making sure his progress reaches whoever reviews the plan, quickly. Progress deserves a plan that keeps up; it does not deserve a freelance experiment on a hard kitchen floor.

*Three.* You arrive to find the regular second carer for Mrs Field's hoisted transfer has called in sick, and the office is quiet. Mrs Field needs the bathroom. — Two-person task, one person present: the transfer does not happen yet, full stop. What does happen: you tell Mrs Field honestly and kindly, keep her comfortable and dignified in the meantime with whatever interim help is within your role, and you get loud — office, on-call number, family, until a second trained person is arranged. Discomfort is real and it matters; it still never justifies a solo hoist. The record afterwards notes the gap and how long it took to fill, because a rota that leaves Mrs Field waiting is a problem someone above you must fix.

**Hold onto these.**

- This course, and any course like it, teaches judgement — techniques are learned in person, from your employer's training, and used only with authorisation.

- A person's full weight never goes through your body; nobody is ever dragged; nobody is moved against their will.

- Know where you are on the spectrum of help, and move rightward only through proper decisions — never through improvisation.

- Two-person means two people. Missing equipment or missing colleagues stop the task and start the phone calls.

- Consent, dignity and the person's own effort are part of every safe move.

- Your back is your livelihood; the "just this once" moment is exactly when to hear this lesson in your head.

Next: the thirty seconds of looking and thinking that come before any move at all — reading the person, the day and the space.
